Functional Job Description

Provides the referrer with a description of the functional requirements of a job/role including work hours, role description, work tasks (cognitive and mental demands), work duties/responsibilities, work environment (physical/psychosocial), critical work demands, work stressors (client and employer perceptions), life activities/hobbies (functional and cognitive demands of these), voluntary work (functional and cognitive demands of these).
If diagnostic formulation, rehabilitation recommendations or an opinion on a person’s work capacity is required please state in referral.
Involves meeting with client, observation of work place where possible, liaison with the employer as appropriate, and reporting.
